You must also complete a Suspected Child Abuse Report (SCAR) form in HealthLink—and send electronically in HL, or fax to (408) 975-5851 PLEASE NOTE: Consulting with a child abuse medical professional through the Children’s Advocacy Center does not replace your responsibility as a mandated reporter to call CPS/Law Enforcement.
