Mandated reporters with a “reasonable suspicion” of child abuse or neglect must call the CPS/DFCS Child Abuse Hotline at (833) 722-5437 and/or call Police where the incident occurred. You must also complete a Suspected Child Abuse Report (SCAR) form in HealthLink—and send electronically in HL, or fax to (408) 975-5851 PLEASE NOTE: Consulting with a child abuse medical professional through the Children’s Advocacy Center does not replace your responsibility as a mandated reporter to call CPS/Law Enforcement.
